How to Fill Out the Costco Cake Order Form
The form has five sections. Here’s what goes in each one and what bakers wish people did:

Before you drop it in the slot: take a photo of the filled form. Custom cakes can’t be returned for a change of mind, so the photo helps if the bakery gets something wrong.

Where to Find and Drop Off the Form
- The kiosk is a white stand near the bakery, usually marked Cake Order Selection, with blank forms, pens and a design book.
- Drop the finished form into the slot on the kiosk, or hand it to a bakery employee.
- At pickup, go to the bakery. Custom cakes are usually in the pickup cooler or behind the counter.
- Pay at any register or self-checkout with your membership card.
Order Form Mistakes to Avoid
- Leaving the phone number blank
- Choosing a pickup day when Costco is closed (Thanksgiving Nov 26, Christmas Dec 25, New Year’s Day)
- Cursive or hard-to-read writing
- Asking for photo prints or characters (not offered)
